Generally, one starts his best five combination, which will most likely will also contain many of the players that are the best at their position on the team. Most of the time, there will be a player in a starting lineup that just can't physically be out on the floor as long as the others and some who seldom need to be taken out. Also, some who can slide over to another position to keep a lineup strong. A "second five" also generally contain some who are not that much of a drop off and some who are a bigger drop off (that's someone you try to "hide" in the lineup). One's opponent also comes in to play as well as the things you mentioned.
