I have a couple of good friends who are very slim and fit, and exercise (running) religiously (one a former wrestler), who have extremely high cholesterol (4x more than I have). They have heart disease, and have to closely watch their diet (processed foods). I know another very skinny person who has extremely dangerous sugar diabetes. I know others who don’t watch their diet at all who seem fine.
I have bad knees but try to exercise by riding my bike 3-5 times a week and 50-75 miles a week. I still had a heart attack. My cardiologist told me to make as much food from scratch, at home and stay away from the ultra-processed foods (lunch meats are a perfect example of ultra- processed food, full of sodium, and other chemicals) in restaurants and grocery stores.
